Enrollment Requirements

Registering your child in a school for the first time can be relatively easy if you are prepared. Following are a few guidelines to help you. Before an enrollment packet is given out, we need to verify your residence. We will need the following:
  1. The custodial parent/s current CA Driver’s License or CA I.D. card (if the Driver’s License or I.D. card show the previous address, the parent will need to obtain an address change from the Department of Motor Vehicles and bring the address change form/card). You can also change your address on the DMV web site and bring the confirmation print out along with your existing license. If you have recently moved from another state or country, please call our office or email for assistance with acceptable documentation.
  2. Two of the following are also required in the custodial parent/s name showing an address within the Grant School boundaries:
    A current gas, electric, or water bill; property tax bill/payment receipts; rental property contract, lease or payment receipts; pay stubs; voter registration card; or correspondence from a government agency.
  3. The student’s birth certificate or passport and immunization records (Please see requirements below)
  4. The student’s last report card and any test scores Grades 1-5

KINDERGARTEN STUDENTS MUST BE 5 YEARS OLD BY SEPTEMBER 1st of the school year for which they are being enrolled.

IMMUNIZATIONS REQUIRED: KINDERGARTEN - BRING IMMUNIZATION RECORDS
POLIO (AT LEAST 3 DOSES IF LAST DOSE IS ON OR AFTER 4TH B-DAY OR 4 DOSES ANY AGE)
DPT (AT LEAST 4 DOSES IF LAST DOSE IS ON OR AFTER 4TH B-DAY OR 5 DOSES AT ANY AGE)
MMR (2 DOSES BOTH GIVEN ON OR AFTER AGE 1)

HEPATITIS B ( 3 DOSES REQUIRED – 2ND DOSE ONE MONTH AFTER 1ST DOSE AND 3RD DOSE 2 MONTHS AFTER 2ND DOSE AND 4 MONTHS AFTER 1ST DOSE)

VARICELLA (1 DOSE REQUIRED FOR KN ENTRY) - UNLESS CHILD HAS ALREADY HAD CHICKEN POX, IF SO WE NEED VERIFICATION FROM YOUR DOCTOR

KINDERGARTEN Medical Exam (CHDP) – All tests must be performed after March 1st of the school year the student will be attending.
